Clinical Assessment

Since undergoing surgery how has the patient progressed? (Choose the answer that most suitably describes the ENTIRE period since surgery)

0 = Not lame | 10 = Holding limb off ground

Please summarise history since surgery. Is there any joint pain on manipulation? Any muscle atrophy? If lameness is present is it intermittent or constant?

Describe the current level of activity: level of confinement; cage/room/backyard, duration and frequency of exercise: walking and/or running, off-lead/on-lead/both, any hydrotherapy, physiotherapy etc.

Please specify dose, duration and efficacy of treatment or write NA if none.
